Oxymorphone is a powerful opioid analgesic used to manage severe pain. It is more potent than morphine (three times), hydrocodone (twice), and oxycodone (twice). This medication is available in immediate-release and extended-release formulations, with effects beginning about 5–10 minutes after injection and 30 minutes after oral administration. Immediate-release tablets provide relief for 3–4 hours, while extended-release tablets last up to 12 hours. Important Warnings:
- Combining Opana with alcohol can increase plasma levels and risk of severe toxicity.
- Take Opana on an empty stomach to avoid a 50% increase in absorption rate caused by food.
Dosage Information:
- For New Users or Those Not Tolerant to Opioids: Start with 5 mg of Opana ER every 12 hours.
- Conversion from Immediate-Release to Extended-Release Oxymorphone: Use the total daily dose of immediate-release oxymorphone, administering half of this dose as extended-release tablets every 12 hours.
- Conversion from Other Oral Opioids: Discontinue all other around-the-clock opioids when starting Opana ER. Use published potency tables or the following conversion factors to estimate the 24-hour oral oxymorphone requirement:
- Hydrocodone, oxycodone, methadone: Conversion Factor (CF) = 0.5
- Morphine: CF = 0.333
- Example: Multiply the total daily dose of the prior opioid by the CF to get the 24-hour oxymorphone dose, then divide by 2 for the starting dose every 12 hours. Adjust as needed.
Side Effects: Seek immediate medical attention for signs of an allergic reaction, such as hives, difficulty breathing, or swelling of the face, lips, tongue, or throat. Serious side effects may include:
- Slowed or stopped breathing
- Light-headedness or fainting
- Seizures
- Chest pain, wheezing, or cough with discolored mucus
- Severe vomiting
- Symptoms of serotonin syndrome: agitation, hallucinations, fever, sweating, shivering, rapid heart rate, muscle stiffness, nausea, vomiting, diarrhea
- Low cortisol levels: nausea, vomiting, loss of appetite, dizziness, fatigue
Older adults and those who are overweight, malnourished, or debilitated may be at increased risk for severe side effects.
Missed Dose: If a dose is missed, skip it if it’s almost time for your next dose. Do not double up on doses.
